Changes to your name

It’s important to let us know of any changes to your personal details so we hold accurate information for you.

If you change your name while you’re registered with us and want to use your new name when you practise, please let us know so we can update the register. We ask you to let us know within one month of the change.

If you choose to keep using your previous name for work purposes, you don’t need to let us know.

If you haven’t legally changed your name, but you want to appear on the register differently from how your name is currently displayed (for example, by adding your middle name), please let us know.

How to change your name

To let us know about your new name, fill a Changing your registered details form.

Read the guidance carefully to make sure you provide all the information we need, in the correct format. There’s no charge for changing your name.

You should also submit a scan or photograph of an official document which confirms the change. For example, a copy of a marriage or civil partnership certificate, a copy of an enrolled deed poll, or a decree absolute or final order.

Please don’t send original documents as we will not be able to return these to you.
